Our Client is currently seeking a Qualified Intellectual Disabilities Professional (QIDP) in the Rogers Park, IL area.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Monitor participants' overall progress; updates programming, as needed and assures that services specified in the services plan are provided in a timely manner.
* Complete all required paperwork accurately and in a timely manner; meeting individual needs, agency requirements, as well as state and federal guidelines. Primary responsibility for ensuring compliance with Rule 115 (CILA), Rule 120 (CILA and Service Facilitation) and the
* Service Facilitation rules for assigned individuals. Convene the CST as required to revise service plans as part of the interdisciplinary process.
* Work collaboratively with all the CILA staff to maximize individual growth and ensure implementation of goals and services identified in the ISP. Work collaboratively with the CILA Site Supervisor to ensure the participation of the DSP's and that each resident's planned goals are achieved.
* Communicate with parents/guardians, peers, staff, and other community resources regularly to maintain positive relationships and ensure service implementation for all individuals on assigned caseload.
* Serve as an advocate on behalf of the individuals. Provides linkage and assistance as necessary to participants/families when establishing/maintaining community services and public benefits for all individuals on assigned caseload.
* Maintain services facilitation claims within budget. Participate in all assigned staff meetings and convene special meetings of the CST when there are issues that need be addressed for all individuals.
* Available for on call CILA issues every other weekend.

REQUIRED SKILLS AND EXPERIENCE
* Minimum of a High School Diploma or its equivalent
* Able to complete and pass State background Check
