Our client is seeking Case Managers to work in the Grand Rapids area earning up to $26/hr
SUMMARY OF THIS OPPORTUNITY:
*Provides intake, screening and direct case management services to clients
*Enforces policies and procedures for delivery of direct services to clients
*Completes documentation to meet standards, grant and donor reporting requirements
*Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation
*Communicate patient needs to a variety of care team members
*Work collaboratively with patients, families, and staff to ensure high quality care
QUALIFICATIONS:
*BA degree; NO LICENSE REQUIRED
*Previous case management experience preferably with either the Mental Health
*Excellent interpersonal and communication skills; verbal and written communication skills
*Ability to respond to common inquiries or complaints from clients, regulatory agencies, or members of the business community
*Ability to effectively interact with individuals from diverse cultures and ethnicities
