SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
This is an excellent opportunity for someone who has Case Management experience in supportive or community housing.
You need to be available for a Full-week (Monday - Friday from 8 to 5) schedule.
A Masters' Degree in a Related Field of Study and previous Case Management or Program Management experience (including 3+ years of supervisory experience) is required.
* Provide direct services to adolescents and foster care families through training and education.
* Work closely with families and early intervention provider staff, to provide education, training, and modeling, connect with services, and assist with daily living skills.
* Collaborate with families and facilities to implement an effective service plan, explaining the available services, including the advantages and disadvantages of each option.
* Communicate and document progress toward recovery.
* Monitor signs and symptoms of psychiatric functioning; assist in community networking and education.
* Collaborate with other providers, agencies and individuals in the client network of care.
CLIENT'S REQUIRED SKILLS AND EXPERIENCE
* A Bachelors' degree is required for this opportunity (preferably in Human Services or another related discipline)
* Minimum of 1 year experience with children and families
* Valid Driver's License
