SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Provide support to individuals by assisting in the development and implementation of support plans.
* Assist individuals in maintaining a safe and healthy lifestyle, and develop a relationship with them to monitor their status.
* Plan and support daily living skills, including household chores, personal care and money management.
* Maintain and complete documentation in a timely manner, ensuring compliance on agency, local and state guidelines and codes.
* Provide mentoring to staff in all areas of service delivery.
* Perform other duties as assigned.
CLIENT'S REQUIRED EXPERIENCE & EDUCATION
* Valid Driver's License
* 1 year experience in DDS and group home management
