Our client is seeking Case Managers to work in the Detroit area
SUMMARY OF THIS OPPORTUNITY:
*Provides intake, screening and direct case management services to clients
*Assisting homeless individuals whom suffer from mental and/or physical disabilities
*Completes documentation to meet standards, grant and donor reporting requirements
* Linking individuals to housing programs and proving case management duties.
*Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation
*Communicate patient needs to a variety of care team members
*Work collaboratively with patients, families, and staff to ensure high quality care
CLIENT'S QUALIFICATIONS:
*AA or BA degree;
*Previous case management experience preferably with either the Homeless Community or Mental Health
*Excellent interpersonal and communication skills; verbal and written communication skills
*Ability to respond to common inquiries or complaints from clients, regulatory agencies, or members of the business community
*Ability to effectively interact with individuals from diverse cultures and ethnicities
