Our client is seeking Case Managers to work in the Denton,TX area. These roles are responsible for assisting clients in the Behavioral Health population in a clinical setting.
ESSENTIAL DUTIES & RESPONSIBILITIES :
• Provides intake, screening and direct Case Management services to clients
• Enforces policies and procedures for delivery of direct services to clients
• Completes documentation to meet standards, grant and donor reporting requirements
• Enters data into the HMIS system, coordinates, tracks and reports activities and outcomes for services provided
• Interacts with clients in a professional manner; keeps client information confidential
• Provides information and referral services
QUALIFICATIONS :
• A four-year degree in social work or a related field of study is preferred but may be substituted by significant relevant work experience
• Previous case management experience preferably with Behavioral Health
• Excellent interpersonal and communication skills; verbal and written communication skills
• Ability to respond to common inquiries or complaints from clients, regulatory agencies, or members of the business community
• Ability to effectively interact with individuals from diverse cultures and ethnicities
• Computer literate and experience with MS Office suite of products
• Bilingual English/Spanish preferred
