One of our clients is seeking Drug & Alcohol Counselors & Assessors to provide supportive services in the Wilmington area.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
*Act as medical case manager for individuals, pregnant women and families affected/infected with HIV/AIDS.
*Manage a caseload of 30-35 clients by assessing needs and implementing service care plans.
*Link clients to appropriate medical,mental health, housing, addiction and support services.
* Attend minimum of 2 medical appointments
*Conduct home visits as necessary
CLIENT'S REQUIRED EXPERIENCE & EDUCATION
* Bachelors Degree Required,
* Ability to work with diverse populations
* Minimum of 1 year of experience in social & human services.
* Ability to complete and pass given clearances including: FBI, state and child background checks, Tuberculosis/TB test, CPR & first aid certification.
