IMMEDIATE OPPORTUNITIES! Our client is seeking a contractor who has experience with individuals coping with HIV/AIDS. Medical Case Managers help HIV-positive clients navigate healthcare and social services, acting as a patient advocate to assess needs, create care plans, link to resources (medical, housing, insurance, transport), coordinate with providers, monitor progress, and ensure access to supportive services for better health outcomes, all while maintaining detailed documentation.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Must have ownership of vehicle
* Ability to manage a caseload of 50 participants.
* Function as a part of the Direct Services team to ensure high quality services are provided to clients.
* Attend weekly supervision with your supervisor and monthly staff meetings.
* Other duties as deemed appropriate by the supervisor/manager.

CLIENT'S REQUIRED SKILLS AND EXPERIENCE
* Degree in Social Work, Human Services, Sociology, Psychology or Nursing. Minimum Bachelor’s Degree, Master’s preferred.
* Valid Driver's License
* Expertise in case management and client advocacy.
* 1-2 years' experience working with HIV/AIDS demographic.
* Computer Proficiency in Microsoft Office Suite required.
* Ability to complete and pass clearances: FBI, Child Abuse, State Background and ACT 31
* Have to have acceptable driving history
