Our client is seeking Blended Case Managers in Philadelphia and the surrounding areas.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Blended case managers provide case management services to children and families with behavioral & mental health concerns.
* Connect clients with appointments & resources to improve daily lives
* Follow up with clients & providers about services received.
* Assist with finding employment, independent living, and improving their overall wellness with appropriate documentation
*Maintain a case load of 25 clients

CLIENT'S REQUIRED EXPERIENCE & EDUCATION
* Bachelors or Masters degree required.
* Minimum one (1) year of case management experience with the mental or behavioral health population.
* Criminal History Clearances - PA State Police Clearance, Child Abuse, FBI, CPR certification, and physical with turberculosis test.
